Physical Therapist (Home Health) – Pay Per Visit
Location: Napa County (Willing to Relocate to Napa County; Opportunities also available in Sonoma County & Santa Rosa)*
Work Setup: Pay Per Visit
Rate: $75 – $85 per visit (based on visit type – Start of Care, Discharge, Routine, etc.)
We are seeking a licensed and motivated Physical Therapist (PT) to join our home health team. In this role, you’ll provide personalized, one-on-one care to patients in their homes — helping them improve mobility, regain independence, and enhance their overall quality of life.

Qualifications
- Active California Physical Therapist (PT) license
