Be the First Connection for Youth and Families
Play a critical role in connecting youth and families to meaningful behavioral health services through OhioRISE. Coleman Health Services is seeking a skilled and compassionate professional to serve as a CME Intake Specialist, acting as the primary point of contact for referrals and ensuring timely, high-quality assessment and access to care. This position is located in our Akron, Ohio, office and serves Summit County.
What You'll Do
- Serve as the primary point of contact for all OhioRISE youth referrals across assigned counties
- Conduct CANS assessments
- Coordinate intake, enrollment, and triaging of referrals to determine immediate needs
- Support youth hospitalized due to behavioral health conditions by assisting with intake and transition planning
- Communicate proactively with referral sources to gather collateral information and provide status updates
- Ensure a smooth, supportive warm handoff to the assigned Care Coordinator following enrollment
- Maintain timely, accurate documentation and tracking of referral and intake activity
Qualifications
- High school diploma or equivalent
- 3 years' experience providing community-based services to youth and their families
- Valid driver's license
- Ability to obtain NPI and Ohio Medicaid numbers (assistance available)
