What are the responsibilities and job description for the Case Manager (Residential) position at Coleman Professional Services, Inc.?
Coleman Health Services is seeking an individual to provide intensive, case management services to individuals with severe and persistent mental health issues in our supportive living environments within Portage County, Ohio. The Case Manager (Residential) will assist with accessing community based resources, housing, employment, making appropriate referrals and linking the individual with ongoing mental health services.
Position Details:
- Day shift working Mondays - Fridays
- Pays $19.24 per hour
- Based in Kent
Qualified candidates of Case Manager (Residential):
- Three years' experience in mental health or related field or BA/BS in mental health or related field
- Have/maintain excellent driving records
- Have or willing to obtain NPI and Ohio Medicaid numbers
- Experience in case management preferred
- Must be able to pass BCI/FBI background checks and an MVR check
