Patient Outreach Specialist (Remote, Part-Time)
Coleman Health Services is building a team of compassionate, service-oriented professionals to support our physician-led patient engagement program.
We believe some of the most important moments in healthcare occur between appointments. Our Patient Outreach Specialists help patients stay connected to their care teams through structured outreach, meaningful conversations, and timely communication.
This is a non-clinical, remote position. No medical experience is required. We are seeking individuals with strong customer service skills, excellent communication abilities, and a genuine desire to help others.
Responsibilities
* Conduct scheduled outreach calls to patients
* Build rapport and maintain professional, compassionate communication
* Gather information using structured questionnaires
* Document interactions accurately
* Escalate concerns according to established protocols
* Work independently in a remote environment
Qualifications
* Minimum 5 years of customer service or client-facing experience
* Excellent verbal communication and listening skills
* Comfortable speaking with people by telephone
* Strong organizational and documentation skills
* Reliable internet connection and dedicated workspace
Preferred
* Veteran
* Retired professional
* Call center experience
* Banking, insurance, hospitality, government service, or administrative experience
* Bilingual skills
