What are the responsibilities and job description for the Home Health/Hospice Licensed Practical Nurse position at Cohesive Healthcare Management and Consulting, LLC?
- Provide nursing care in accordance with the client’s plan of care to include comprehensive health and psychosocial evaluation, monitoring of the client’s condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care.
- Assist the Registered Nurse in carrying out the plan of care. Provides skilled nursing services in accordance with the orders of a physician and under the plan of care in patient’s home. Responsible for the observation, assessment nursing diagnosis, counsel, care and health teaching of the ill, injured or confined and the maintenance of health and prevention of illness.
- Records the health needs of the patient.
- Coordinates the care of all patients in his/her case load.
- Makes referrals to other agencies or services as needed.
- Participates in in-service programs, staff meetings and weekly Case Conference meetings.
- Complies with Home Health Compliance Plan, Infection Control and HIPPA policies.
- Available to work after hours and weekends on an on-call basis, per policy to meet the client car requirements.
- Licensed Practical Nurse in the State of Oklahoma
- Home Health Care experience preferred
- Excellent communication and time-management skills with ability to work independently
- Knowledge of regulatory standards and compliance
- Working Knowledge of computer and software applications used in job function (Word, Excel, Outlook and EMR software)
- Current Automobile insurance coverage
- Workable knowledge of Medicare, CHAP accreditation criteria and standards
- Willing to be on call as assigned
