Position Summary:
Codman Square Health Center (CSHC) seeks an experienced nursing leader to serve as Director of Care Management & Workforce Development. The Director of Care Management & Workforce Development provides strategic and operational leadership for care management, community health, and workforce development functions across the organization. This role is responsible for advancing population health, care coordination, and non-visit-based models of care in alignment with FQHC priorities and value-based payment models.
Organizational Leadership & Reporting Structure:
The Director of Care Management & Workforce Development supervises:
- Nurse Supervisor – Care Management
- Nurse Supervisor – Clinical Education
- Community Health Workers (CHWs)
The Director of Care Management collaborates closely with the Director of Nursing, Medical Directors, Operations, Quality, Compliance, and Data teams to support integrated, team-based care delivery.
Primary Responsibilities:
- Care Management & Population Health
Leads development and oversight of care management programs supporting high-risk, high-utilizing, and complex patient populations. Ensures effective care coordination, transitions of care, and integration of community-based resources.
- Community Health & Workforce Development
Oversees Community Health Worker programs and workforce development initiatives that support patient engagement, outreach, and health equity.
- Clinical Education & Infection Prevention
Provides leadership oversight for clinical education and infection prevention functions in partnership with nursing and operations leadership.
- Value-Based Care & Performance Improvement
Uses data to monitor care management outcomes, utilization, and quality metrics tied to value-based contracts, capitation, and shared savings models.
- Leadership & Staff Development
Recruits, mentors, evaluates, and supports supervisors and frontline staff. Ensures standardized onboarding, training, and competency development across care management and education teams.
- Collaboration & Strategic Leadership
Represents care management in organizational planning, committees, and strategic initiatives. Partners with executive leadership to advance population health strategy.
- Performs other duties as assigned.
Required Qualifications:
- Bachelor's degree
Preferred Qualifications:
- Experience in health centers, hospital and/or academic administration.
Physical Requirements:
- Must be able to sit or stand for prolonged periods of time.
- Must be able to lift 10 pounds and load onto shelves.

Salary : $127,500 - $198,500
