Cobble Hill Health Center is a not-for-profit long and short-term care facility providing services to the disabled and elderly for over 40 years.
The CNA plays an integral role in helping us achieve our vision: to consistently exceed expectations and set the standard for excellence in residential and community care.
Duties & Responsibilities:- Assist residents with activities of daily living, including delivering meals, turning and ambulating residents, and ensuring they receive adequate fluids and nourishment throughout the shift.
- Assist residents with personal hygiene and grooming.
- Observe residents and report any health concerns to the supervising nurse.
- Document resident food and fluid intake.
- Assist residents with feeding and nutritional supplements.
- Make residents' beds and keep living areas clean and tidy.
- Transport residents throughout the facility.
- Assist nurses and other staff members as needed.
- Adhere to professional standards and all facility policies and procedures.
- Comply with all federal, state, and local regulations.
- Maintain the confidentiality of all resident and facility information.
- Active New York State Certified Nurse Assistant (CNA) certification.
- High School Diploma or equivalent.
- Strong English reading, writing, and communication skills.
- Prior CNA experience is a plus.
- Skilled nursing facility experience preferred but not required.
- SigmaCare experience is a plus.
