Social Worker

Cobble Hill Life Care is a not-for-profit long- and short-term care facility in Brooklyn, NY, dedicated to providing compassionate care and support to older adults and individuals with disabilities. We are seeking a compassionate, organized Social Worker to support residents and their families throughout their care journey. In this role, you’ll help residents adjust to care, understand their options, and stay connected to the people and resources that matter most.

What You’ll Do

• Complete psychosocial assessments and contribute to individualized resident care plans.
• Support residents and families through admissions, care transitions, and discharge planning.
• Provide counseling, advocacy, and emotional support to residents and their families.
• Collaborate with nursing, medical, rehabilitation, and other interdisciplinary team members to promote resident well-being.
• Help residents and families navigate facility processes, community resources, and available services.
• Maintain accurate documentation and follow up on resident and family concerns in accordance with facility policies and applicable regulations.
• Advocate for residents’ rights, dignity, independence, and quality of life.

Qualifications & Requirements

• Bachelor’s degree in Social Work (BSW) or a related field required; Master’s degree in Social Work (MSW) preferred.
• New York State social work licensure or registration, if applicable to the position.
• Previous experience in a skilled nursing facility, long-term care, healthcare, or a related setting preferred.
• Strong communication, interpersonal, organizational, and problem-solving skills.
• Ability to work compassionately with older adults, individuals with disabilities, and their families.
• Knowledge of psychosocial assessments, care planning, discharge planning, and community resources preferred.
• Ability to collaborate effectively with an interdisciplinary care team.
• Strong documentation skills and attention to detail.
• Commitment to resident rights, confidentiality, dignity, and quality of care.
