Per Diem RNA, needed to come to our small six bed congregate living facility once or twice a week as needed. Flexible days and hours available.

Responsibilities
- Implement and monitor individualized restorative care plans focused on enhancing residents’ mobility, strength, and daily living skills.
- Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and transferring using proper techniques and equipment such as Hoyer lifts.
- Document resident progress accurately in electronic health records (EHR/EHR systems) while maintaining strict compliance with HIPAA regulations to ensure confidentiality.
- Collaborate with nursing staff to assess resident needs, report changes in condition, and adjust care strategies accordingly.
- Support infection control protocols by practicing aseptic techniques during procedures like catheter care and wound management.
- Educate residents and their families about restorative programs to promote engagement and understanding of care goals.
- Utilize patient transfer methods safely, including manual handling practices, to prevent injury for both residents and staff.
Requirements
- Experience working with patients in long-term care facilities, assisted living communities, or similar settings.
- Knowledge of medical terminology, vital signs measurement, and basic life support (CPR) certification.
- Demonstrated ability to communicate effectively with patients, families, and interdisciplinary teams in a compassionate manner.
- Ability to perform heavy lifting safely and adhere to safe lifting practices during transfers or repositioning tasks.
