Coastal Hospice invites you to join a team of caregivers who are committed to promoting dignity and quality of life for patients and families who face life-limiting conditions. In your role as an RN Case Manager, you will:
- Coordinate with patient/family and Interdisciplinary Team to implement and manage all aspects of patient/family hospice care including physical, spiritual, psychosocial, financial and bereavement needs in the patient’s home or inpatient unit
- Be responsible for developing, implementing and evaluating a plan of patient care utilizing ANA (American Nurses Association) Standards and the Nursing Process
- Utilize equipment and technology skillfully and is technically competent in nursing procedures
- Oversee and/or participates in safe medication handling and practices
- Educate patients and families on significant self-care and end of life issues, all hospice services, patient liability for payment and ensures consent and payer forms are completed
- Act as a patient advocate. Provides support, information and direct care
- Triage phone calls to identify patient/family needs and notifies Admission Services of referrals
- Oversee and delegate assignments of CMTs, LPNs, Hospice Aides, students and volunteers on shift
Qualifications
Education and Work Experience Requirements:
• Licensed and currently registered to practice as a Registered Nurse in the State of Maryland, Delaware or other compact state
• BSN from an accredited college preferred
• CHPN certification expected within two (2) years of employment

The pay rate/range listed represents Coastal Hospice's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
Salary : $68,000 - $90,000
