Role Description
This is a full-time, on-site role for a Qualified Professional located in Elizabethtown, NC. The Qualified Professional will be responsible for providing case management, coordinating care for clients, conducting assessments, creating and implementing treatment plans, and working collaboratively with interdisciplinary teams. Daily tasks include engaging with clients, documenting progress, and ensuring compliance with regulatory standards. The role requires strong communication and problem-solving skills to effectively support clients' needs.
Qualifications
- Case Management and Care Coordination skills
- Experience in conducting assessments and creating treatment plans
- Strong communication and interpersonal skills
- Ability to work collaboratively with interdisciplinary teams
- Understanding of regulatory compliance and documentation standards
- Experience in mental health, substance use, or crisis intervention is a plus
- Bachelor's degree in Social Work, Psychology, Counseling, or related field
- Relevant certifications or licensure (e.g., LCSW, LCAS) preferred
