Nurse
Job Summary: Part of a small team of nurses that supports all the health care needs for persons with disabilities attending Coastal Connections Day Habilitation Program. Individuals require support for dining/feeding, medication administration and ongoing health concerns often related to those disabilities. This entails following outlined plan of care, identifying health-related needs, monitoring medical status and reporting changes to Health Care Supervisor, and collaborates with residential/health care team/family/guardians to address identified needs.
Major Responsibilities:
- Health Monitoring & Care:
- Conducts regular health assessments, including vital signs and symptom checks, to monitor individual health & determine nursing needs. Reports to and seeks direction from the Health Care Supervisor.
- Medication Management –Administers medications and treatments as prescribed per orders. Manages medication storage and documentation in compliance with regulatory standards.
- Demonstrates competence in all procedures.
- Care Plan Implementation:
- Understands clinical and social needs of individuals receiving services.
- Assists in the development and implementation of individual specialized care plans.
- Completes and maintains written records. Prepares and submits all paperwork in a timely manner. Including but not limited to daily data collection, incident reports, and progress notes.
- Utilizes nursing knowledge and skills to safely implement basic preventative care and therapeutic and rehabilitative interventions.
- Medical Coordination & Communication:
- Communicates any change in an individual's status, orders or plan of care to the Health Care Supervisor.
- Presents information regarding any individual in a professional manner in all settings including meetings, phone contacts, daily written documentation and/or email correspondences.
- Follows HIPAA and security regulations and/or guidelines to maintain confidentiality at all times.
- Participates in supervision, general staff meetings, and training as scheduled.
- Compliance:
- Maintains universal precautions at all times and implements all Standards related to Health and Safety.
- Maintains a license in good standing and current BLS.
- Demonstrates and implements knowledge Policies and Procedures, including Standards of Conduct.
- Ensures that the human, civil and legal rights of all individuals served are protected.
- Teaches and promotes dignity and the right of choice for all individuals & their families.
Qualifications:
- Licensed LPN or RN in Massachusetts with minimum of 1 year of experience
- Experience working with individuals with intellectual or developmental disabilities preferred
- Ability to develop and maintain a respectful, caring, tolerant, ethical and empowering relationship with individuals with intellectual and physical disabilities
- Must maintain CPR certification and complete required programmatic trainings
