Position Overview:
We are seeking a warm, organized, and detail-oriented First Impressions Coordinator to join our client focused care team. This person will play a key role in ensuring our clients and their families have a seamless, personalized, and consistently positive experience with Coastal Care Partners.
Key Responsibilities:

Serve as a primary point of contact for our medical and home care clients.

Proactively check in with clients to ensure satisfaction and quickly address any concerns

Manage care transitions and updates, including changes in client condition or care plan

Maintain accurate client records and communication notes in multiple systems

Support the onboarding of new clients and help create personalized care experiences
- Collaborate closely with medical providers, leadership, caregivers and internal teams to ensure quality and consistency

Bluffton, SC (in-office)
Schedule:
Full-time | Monday–Friday | Occasional on-call or after-hours support as needed
