- Job Details
- Provide direct patient care services to hospice patients and supportive care to the patient’s family.
- Manage an interdisciplinary group including Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance hospice care quality.
- Case manage all assigned patient clinical records.
- Assure highest quality of care is provided and documented according to the Plan of Care.
- Perform initial and ongoing patient assessments.
- Develop and implement Hospice Plan of Care as prescribed by physician and interdisciplinary group.
- Work closely with interdisciplinary group to provide physical, psychosocial, educational, emotional, and spiritual services including bereavement support.
- Report any change in patient condition to Attending and Hospice Physicians.
- Maintain accurate and updated medical records including interdisciplinary notes and plans of care.
- Practice infection control measures and standard precautions.
- Monitor scheduling of all staff.
- Participate in Quality Assessment and Performance Improvement Program.
- May participate in agency on-call schedule.

- Complete required Compliance annual training.
- Attend staff meetings.
- Maintain high knowledge of pain and symptom management in dying process.
- Make routine supervisory visits with team members monthly.
- Perform ongoing review and follow-up on patient clinical records to assure quality and compliance.
- Maintain close contact with patient’s family or significant others for information, support, and continuity of care.
- Maintain primary control and professional management of each patient acting as liaison between physician, patient/family, and hospice team.
- Responsible for education of patient, family, or caregivers on Plan of Care needs including medication administration, dressing changes, oxygen administration, hospice philosophy, and care of terminally ill patient.
- Job Requirements
- Minimum of one (1) year experience as an RN in acute care hospital, oncology, long-term care, hospice, or home health (school internships and clinical hours may be considered).
- Knowledge of terminally ill patients and their families and hospice special needs.
- Knowledge of roles of all disciplines providing hospice services.
- Excellent patient assessment skills.
- Knowledge of community resources.
- Good oral and written communication skills.
- Documentation and program management skills.
- Knowledge of general nursing practice and managed care principles.
- Familiarity with regulatory guidelines (Medicare, Medicaid, ACHC).
- Human resource management principles.
- Ability to operate nursing assessment tools, computers, facsimile equipment, copier, and cell phone/beeper.
- Must have and maintain transportation for work.
- Registered Nurse license in the state of employment.
- Current automobile insurance and valid driver’s license.
