Duties

• Conduct comprehensive patient assessments via phone or electronic communication, including vital signs, medical history, and current symptoms
• Provide expert triage by evaluating patient needs based on physiology knowledge, medical terminology, and clinical guidelines
• Collaborate with healthcare professionals across hospital medicine, emergency medicine, behavioral health, infectious disease care, and post-acute services to determine appropriate interventions
• Manage patient intake and discharge planning processes to ensure seamless transitions of care in inpatient, outpatient, home health, and hospice settings
• Document all interactions accurately within Electronic Health Records (EHR) systems while adhering to HIPAA regulations to protect patient privacy
• Facilitate diagnostic evaluation referrals and coordinate case management efforts for complex cases involving ICU, PICU (Pediatric Intensive Care Unit), Geriatrics, or Level I/II trauma center experiences
• Support utilization management initiatives by assessing medical necessity and managing patient care plans in compliance with health regulation policies

Experience

• Proven clinical experience in critical care environments such as ICU, emergency room, or Level I/II trauma centers
• Extensive background in hospital medicine, medical-surgical units, or outpatient clinics with a focus on acute care and patient assessment
• Demonstrated expertise in managing diverse patient populations including pediatrics (PICU), geriatrics, behavioral health patients, and those requiring infectious disease care
• Familiarity with electronic health records (EHR/EHR management), medical records documentation, discharge planning, and case management systems
• Strong knowledge of physiology principles, diagnostic evaluation procedures, and medical terminology essential for accurate triage decisions
• Experience working within managed care settings or utilization management programs to optimize patient outcomes while controlling costs
• Excellent client communication skills paired with the ability to collaborate effectively with multidisciplinary healthcare teams to deliver exceptional patient service
