Job Overview
We are seeking a dedicated and compassionate Case Manager/Supportive Counsleor to join our team. The ideal candidate will have a strong background in mental health counseling and a passion for helping individuals and families navigate complex challenges. This role involves providing comprehensive case management services, including assessment, planning, and coordination of care for clients with diverse needs, including children, individuals with developmental disabilities, substance abuse and those experiencing crises. This person will be working in our sober living home in Huntington, West Virginia and occasionally at our office in Fort Gay, West Virginia. Most of the work will be in the Huntington.

Duties

  • Conduct thorough patient assessments to determine individual needs and develop tailored care plans.
  • Provide individual and group counseling services utilizing techniques such as cognitive behavioral therapy, motivational interviewing, and crisis intervention.
  • Collaborate with multidisciplinary teams to coordinate patient care and ensure effective treatment strategies are implemented.
  • Maintain accurate medical documentation in compliance with HIPAA regulations, including ICD-9 and ICD-10 coding.
  • Facilitate access to community resources and support services for clients and their families.
  • Engage in direct support activities for clients, including behavioral management and crisis management.
  • Conduct intake assessments for new clients to evaluate their mental health status and treatment needs.
  • Implement therapeutic interventions such as art therapy, dance therapy, or psychodynamic therapy as appropriate.
  • Monitor client progress through regular follow-ups and adjust care plans as necessary.
  • Participate in clinical research initiatives to enhance service delivery and improve patient outcomes.
  • Teach our DUI Class

Qualifications

  • A bachelor's degree in Social Work, Psychology, Counseling, or a related field is preferred.
  • Experience working with substance abuse, children and families is highly desirable.
  • Familiarity with behavioral health practices, including PTSD care, addiction counseling, grief counseling, and chronic pain counseling.
  • Knowledge of medical terminology and experience with medical records management is essential.
  • Proficiency in telehealth practices is a plus; experience in outpatient or inpatient settings is preferred.
  • Strong understanding of special education needs and experience working with individuals with autism or other developmental disabilities is beneficial.
  • Excellent communication skills with the ability to build rapport with clients from diverse backgrounds.
  • Ability to work collaboratively within a team-oriented environment while also demonstrating independence in managing caseloads.
