The IDDT Case Manager provides integrated, community-based case management services and supports to individuals with co-occurring mental health and substance use disorders. This role focuses on promoting recovery, independence, and overall wellness through evidence-based practices, person-centered planning, and stage-wise interventions.

Compensation Range: 

The starting pay for this position is $44,000 - $65,770 for unlicensed candidates, $50,800 - $70,542 for limited licensed candidates, and $52,593 - $75,315 annual salary for fully licensed candidates, and is based on non-discriminatory factors such as skills and experience.

  • Provide community-based case management and integrated behavioral health services tailored to individuals with co-occurring disorders.
  • Conduct assessments addressing mental health, substance use, physical health, housing, employment, and social supports.
  • Develop and implement individualized Person-Centered Plans in compliance with OCHN, DWIHN, and accrediting body standards.
  • Deliver stage-wise interventions for co-occurring disorders using Motivational Interviewing, Harm Reduction, and Integrated Treatment principles.
  • Provide weekly community-based interventions and monitor individual progress in recovery.
  • Facilitate individual and group interventions focused on relapse prevention, coping skills, and recovery support.
  • Assist individuals with accessing and navigating community resources, including housing, education, vocational programs, and peer support networks.
  • Coordinate care with primary care physicians, psychiatrists, substance use specialists, and other providers during regularly scheduled IDDT team meetings.
  • Provide education to individuals and families on symptoms of co-occurring disorders, recovery
    strategies, and medication management.
  • Monitor adherence to medication regimens, support medication coordination, and provide education regarding side effects.
  • Complete timely, accurate documentation in compliance with CNS policy and funding source requirements.
  • Conduct hospital pre-screenings and assist in transitions of care from inpatient to community settings.
  • Use Trauma-Informed Care principles to support recovery and minimize re-traumatization.
  • Advocate for individuals to ensure access to services, supports, and protections of rights. 

 

Qualifications:

Required   

  •  Qualified Mental Health Professional (QMHP) with one or more of the following degrees: Social Work, Psychology, Sociology, or Criminal Justice.
  • Knowledge and experience working with co-occurring mental health and substance use disorders. 
  • Ability to apply evidence-based practices for integrated treatment of dual disorders.
  • Proficiency in Motivational Interviewing and stage-wise approaches to treatment.
  • Knowledge of psychiatric medications, side effects, and adherence strategies.
  • Strong documentation, organizational, and time management skills.
  • Effective communication and interpersonal skills with individuals, families, and professional partners.
  • Ability to work independently and as part of a multidisciplinary team.
  • Knowledge of applicable Federal, State, and Local regulations, CNS Corporate Compliance Program, and CNS Code of Ethics.     

 Preferred

  • One year paid or supervised volunteer experience in health or human services preferred.
  • CADC/CAADC Credentials or be willing to submit a development plan with MCBAP within 6 months of hire.
  • State licensure preferred; LLBSW, LBSW, LLMSW, LMSW.
  • Advanced IDDT training, if not applicable, will be required to complete certificates in Basic and Advanced IDDT training.

  Additional Eligibility Requirements:

  • Valid and unrestricted Driver’s License
  • Automobile accessibility with current automobile/vehicle insurance meeting agency policy requirements.
