Job Overview
We are seeking an advanced practice provider(NP/PA-C) to join our healthcare team. The ideal candidate will have a strong background in evaluating and treating patients with acute and chronic conditions in an outpatient setting with prior 3 years experience in adult primary care. We intentionally limit schedules to approximately 15–18 patients per day so our providers can deliver high-quality, patient-centered care without sacrificing work-life balance.
Responsibilities
- Conduct comprehensive patient assessments, including obtaining medical histories and performing physical examinations.
- Diagnose and treat a wide range of acute and chronic medical conditions under established clinical protocols and physician supervision.
- Develop, implement, and monitor individualized treatment plans based on evidence-based clinical guidelines.
- Order, interpret, and communicate laboratory tests, diagnostic imaging, and other diagnostic studies.
- Prescribe medications and other therapies in accordance with state regulations and organizational policies.
- Perform office-based procedures within the scope of practice, including wound care, joint injections, skin biopsies, and other minor procedures as appropriate.
- Provide preventive care services, including annual wellness visits, health screenings, immunizations, and patient education.
- Monitor patients with chronic diseases such as diabetes, hypertension, hyperlipidemia, chronic kidney disease, and COPD, adjusting treatment plans as needed.
- Educate patients and families regarding diagnoses, treatment options, medications, lifestyle modifications, and disease prevention.
- Coordinate patient care with physicians, specialists, nurses, pharmacists, and other healthcare professionals to ensure continuity of care.
- Document patient encounters accurately and timely in the electronic health record (E-clinicalworks), ensuring compliance with regulatory and organizational standards.
- Review and respond to patient messages, laboratory results, imaging reports, medication refill requests, and other clinical communications.
- Participate in quality improvement initiatives, population health management, and performance improvement activities.
- Maintain current knowledge of clinical guidelines, best practices, and continuing medical education requirements.
- Adhere to HIPAA regulations, organizational policies, and all applicable federal and state laws governing physician assistant practice.
- Perform other clinical duties as assigned to support the mission and goals of the practice.
Qualifications
- Must hold a valid Physician Assistant or Nurse Practitioner license.
- Expert level in using electronic health record systems for documentation and typing
- 3 years of experience in similar setting and proven track record of staying at a job long term.

Pay: $110,000.00 - $140,000.00 per year
