Requirements
- Up to 18 patients per caseload
- 1 year of home health or hospice experience
- Coordinate care for the patient among the nursing team to include RNs, LPNs, and HHA.s, as well as complete routine visits and as needed visits.
- Complete comprehensive assessment of patients identify their care needs, and determine appropriate goals and interventions to meet those needs.
