Title: West Bridge Family Case Worker
Reports to: Housing Director
Job Purpose
West Bridge Apartments operates as Permanent Supportive Housing (PSH). The purpose of PSH is to connect households with the appropriate long-term supports (in-house and community based) so they can transition to alternative housing beyond West Bridge (no specific time frame). West Bridge target demographic consists of homeless, chronically homeless, disabled, and veteran preference. The Case Manger is a position that supports activities at West Bridge Apartments by identifying needs and solutions for residents, track outcome data on each household served, and participate in continuous quality improvement activities.
Duties and Responsibilities
· In coordination with the Housing Director implement, define, and expand the delivery of case management services to current West Bridge Residents.
· Implement agency defined assessment processes and follow defined protocols to serve residents through the appropriate service track within the utility and housing programs.
· Implement the current procedure/plan that addresses each household’s needs, and upon approval, assist the residents by engaging them toward self-sufficiency.
· Routinely evaluate the progress toward goals identified in each plan of service and recommend plan modification as needed to encourage ongoing progress toward self- sufficiency and defined program outcomes. Assessments completed at move-in, monthly, and annually; SPDATs at move-in, 2 weeks after move-in, 30 days, 60 days, 3 months, 6 months, 9 months, and annually. After the 1st year SPDATs are to be completed annually.
· Case Management/Direct Client Services: scheduled and spontaneous meetings and interactions with clients. Assisting them with everyday necessities, transportation, etc.
· Recommend resident referral to suitable resources and agency partners. Use problem-solving skills to navigate household barriers to self-sufficiency.
· Coordinate with appropriate agencies for referrals: CMH, Tribal, Veteran, Housing, Child Services/Needs, Food, Healthcare, Life Skills, Social Security, Disability, etc.
· Identify client-based needs and arrange for life skill classes within the community and community based.
· Develop a professional and supportive relationship with each resident that establishes trust and serves to motivate toward self-sufficiency.
· Advocate on behalf of residents and resident rights at local, county-wide, and state platforms.
· Network with community-based programs and agencies to bring in services, programs, and resources to the residents.
· Use skills and experience to develop and implement housing and service plans within program criteria and grant guidelines. Utilize MSHMIS data base to create housing plan(s), goal(s), etc. All data (assessments, interims, client interactions, services provided, needs identified, case notes, etc.) is to be entered in HMIS immediately. Assessments should be completed live as much as possible.
· Run/use established reports (HMIS Data Quality, CSV, APR, CoC, etc.) from HMIS (Service Point) and/or other relational databases to recommend continuous quality improvement processes to improve program outcomes.
· Be thoroughly familiar with approved assistance grants, their conditions, and eligibility criteria.
· Work closely with other staff members to ensure data quality.
· Create reports, business correspondence and procedure manuals as assigned.
· Travel independently to areas served by C-L-M Community Action Agency, with appropriate compensation.
· Attend staff meetings and Local Planning Board Meetings to identify services within the community.
· Meet with Housing Director as scheduled to review files for service plan coordination, organization, etc.
· Network within the agency and out in the community.
· Assist Site Manager with Meetings to address issues and changes.
· Occasional planning of fun events for residents.
· Supervision of occasional volunteers.
· Other duties as assigned.

· Possess and demonstrate skill and experience in effectively coordinating multiple projects with short deadlines.
· Ability to work with a variety of systems, agencies, data and individuals while exhibiting excellent communication skills.
· Ability to maintain records and prepare verbal and written reports.
· Ability to explain program policies and procedures to consumers and families of consumers.
· Ability to establish and maintain effective working relationships with administrators, employees, consumers, and families of consumers.

Required Qualifications
High School Diploma or equivalent required.
Bachelor’s degree preferably in social work, human services, community development, business, or a related field. Or, the equivalent education and experience in the human service field. Prior experience working in the human services field preferred.
Will be required to pass criminal history check.
Must have valid Michigan driver’s license, an acceptable driving record and reliable transportation.
