Role Description
Clinician Link NW (CLNW) is hiring Physical Therapists to work with our partnered home health agencies in the area. With CLNW, you can work as much or as little as you want, we will prioritize your needs and desired hours.

We will make sure you have the support you need to get setup with our partner agencies. These visits include home health admission visits, follow up visits, PT evals, and discharges.

Clinician Link NW allows home health agencies to say yes to every referral. It was created by two PTs in the Portland area after watching agencies struggle to manage the ebb and flow of referral volume.

Qualifications
- Preferred: Home Health experience
- Required: Licensed as a Physical Therapist in the state of Oregon
- Ability to work independently and communicate effectively with patients, caregivers, and healthcare teams
- Familiarity with home health documentation standards and compliance
