The Director of Home Health leads a high-performing home health agency, driving clinical excellence, regulatory compliance, and growth while ensuring exceptional patient outcomes and satisfaction. This role oversees day-to-day operations, including staffing and scheduling of interdisciplinary teams, intake and referral management, care coordination, and documentation integrity within the EMR. Daily responsibilities include running morning huddles, reviewing census and visit productivity, assigning caseloads, guiding case conferences, approving physician orders, monitoring KPIs (hospitalizations, timeliness, Star Ratings), and conducting chart audits for quality assurance and performance improvement. The Director Home Health partners with physicians and referral sources to grow volumes, maintains survey readiness (CMS Conditions of Participation; Joint Commission/ACHC/CHAP), manages budgets and supply costs, mitigates risk and infection control, mentors and onboards staff, implements evidence-based protocols and telehealth workflows, and elevates patient experience through proactive communication, service recovery, and continuous process improvement—positioning the agency as a trusted, compliant, and results-driven home health care provider.

What's Needed To Succeed
- Degree from an accredited School of Medicine or Nursing Program
- Licensed as a Physician or Registered Nurse in the state of Texas or ability to obtain
- Prior leadership experience in a home health setting
