As a Case Manager, you will play a vital role in ensuring our patients receive seamless, coordinated care from admission through discharge and beyond. You’ll collaborate with healthcare providers, patients, and their families to develop care plans, connect them to necessary resources, and facilitate optimal health outcomes. Responsibilities include coordinate and oversee patient care plans in collaboration with medical staff, patients, and families; perform assessments to identify patient needs, including social, emotional, and financial support; facilitate discharge planning and ensure continuity of care by coordinating post-discharge services; act as a liaison between patients, families, and the healthcare team to address concerns and resolve barriers to care.
