This is a DIRECT HIRE OPPORTUNITY (not a contract).
Key Responsibilities:

• Provides clinical physical therapy evaluation and treatment plans for program participants 55 and older, on an acute restorative or maintenance level as needed
• Responsible for integrating physical therapy treatment plans into the Plan of Care (POC) as directed by the physician's orders.
• Guide and supervise patients in performing therapeutic exercises and techniques
• Track patient progress, document treatment outcomes and progress in medical records, and make necessary adjustments to the treatment plan
• Occasionally assist patients with DME set-up
  

Key Requirements:

• Bachelor’s Degree in Physical Therapy required, Master's in Physical Therapy preferred
• Bilingual preferred
• Completion of a physical therapy program accredited by the American Physical Therapy Association (APTA), the Committee on Allied Health Education and Accreditation of the American Medical Association, and the American Physical Therapy Association or other equivalent organization approved by CMS
• Current, active Physical Therapist license within the state of practice
• A minimum of one (1) year of experience working with the elderly
• Bilingual is a huge plus
  

Compensation and Schedule:

• Annual pay starts at $93K but can be considerably higher based on years of experience
• $5K Sign On Bonus!!
• Comprehensive benefits plan
• Monday-Friday, 8 AM - 4:30 PM
• Paid holidays and NO weekends
