Physical Therapist – Home Health
Clinical Magnet, a division of ICON Medical Network, is seeking a compassionate and motivated Physical Therapist for a full-time, permanent Home Health opportunity in the New Haven, CT area - coverage flows from New Haven to Branford, Guilford, Madison, and the CT shoreline. Position Highlights
- Guaranteed salary structure, $90,000-$110,000/year based on experience

Responsibilities
- Evaluate and treat patients in the home health setting
- Develop individualized treatment plans based on patient needs and goals
- Monitor patient progress and adjust care plans as appropriate
- Collaborate with physicians, nurses, and interdisciplinary care teams
- Complete timely and accurate documentation
- Educate patients and caregivers on therapeutic techniques and safety
Qualifications
- Graduate of an APTA-accredited Physical Therapy program
- Active Connecticut PT license
- Minimum of 1 year of clinical experience preferred
- Home health experience preferred but not required
- Valid driver’s license, reliable transportation, and current auto insurance
- Ability to travel throughout the assigned territory
