Clinical Magnet, a division of ICON Medical Network, is hiring a dedicated Physical Therapist for a full-time permanent Home Health opportunity covering the Waterbury, Connecticut territory. Responsibilities

• Provide skilled physical therapy services to patients in their homes
• Perform evaluations and establish individualized treatment plans
• Assess functional deficits and monitor patient progress
• Coordinate care with physicians and interdisciplinary team members
• Maintain timely and accurate documentation
• Educate patients and families on exercises, mobility, and safety
  

Qualifications

• Graduate of an accredited Physical Therapy program
• Active Connecticut PT license in good standing
• Minimum of 1 year of experience in a healthcare setting
• Home health experience is a plus
• Reliable transportation, valid driver’s license, and auto insurance required
• Ability to travel within the branch coverage area
