RN Case Manager Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans
- Provide skilled nursing visits in accordance with physician orders
- Monitor patient progress and evaluate physical, psychosocial, environmental, and caregiver needs
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals
- Communicate changes in patient condition and recommend appropriate interventions
- Educate patients and caregivers regarding treatment plans and disease management
- Maintain accurate and timely clinical documentation
- Ensure compliance with regulatory standards and patient confidentiality requirements
Qualifications
- Current, active, unencumbered Registered Nurse (RN) license
- Minimum of one year of nursing experience
- Valid driver's license, reliable transportation, and auto insurance
- Ability to travel throughout the assigned territory
- Home Health experience preferred
- Strong communication, organization, and case management skills

- Salary range of $85,000 - $98,000 based on experience
