Job Summary:
The Case Management Director oversees the day-to-day operations of the Case Management department, ensuring effective care coordination, utilization management, and discharge planning. This is a great opportunity for an experienced healthcare leader ready to step into a high-impact role.
Key Responsibilities:
- Manage daily operations of the Case Management department
- Oversee utilization review and discharge planning processes
- Ensure compliance with policies, regulations, and quality standards
- Lead, mentor, and support Case Management staff
- Collaborate with clinical and administrative leadership teams
- Assist with patient caseloads when needed
- Drive performance, efficiency, and patient outcomes
Qualifications:
- Bachelor’s degree required (healthcare-related field preferred)
- Minimum 3 years of hospital-based case management leadership experience
- Strong background in utilization review and discharge planning
- Acute care or inpatient rehabilitation experience preferred
- CCM or ACM certification preferred (required within first year if not already obtained)
- Demonstrated leadership stability (2 years in prior roles preferred)
