Medical Coder

Title of Position: Medical Coder

Position Summary:

The Medical Coder will be responsible for coordinating/supporting retrospective and concurrent chart reviews using knowledge of ICD10-CM and CPT coding to translate, input, extract and validate medical record data.

Responsibilities:

• Review patient medical history and physical exams, physician orders, progress notes, consultation reports, diagnostic reports, operative and pathology reports, and discharge summaries in order to verify whether the diagnosis codes are supported by the documentation to ensure they are within Guidelines for Coding and Reporting
• Review clinical documentation and assign accurate diagnosis codes according to guidelines and projects.
• Assist in obtaining medical records from previous physicians
• Review and advising about HEDIS measures
• Ensure compliance with all applicable Federal, State and/or County laws and regulations related to coding and documentation guidelines.
• Educate Physicians regarding proper billing and documentation policies, procedures, and conflicting/ambiguous or non-specific documentation
• Prepares and submits to the supervisor the reports required.
• Prepares and submit claims from the visit
• Maintaining strictest confidentiality and adhering to all HIPAA guidelines and regulations
• Ability to maintain effective working relationships with employees.
• Performs other duties relevant to the position

Qualifications:

• CPC /CRC/CPMA/CDEO certification
• Minimum 1 year of experience in Medical coding
• Advanced knowledge of Microsoft suite (Word and Excel)
• Familiar with HCC Dashboard tool
• Strong knowledge of ICD-10, CPT and HCPCS codes
• Fluent in English and Spanish

Job Type: Full-time

Pay: $18.00 - $25.00 per hour
