Job Requirements
ARRT Certification
Primary Source Verification of professional license and / or certification, as required for the position ran within 30 days of start updated upon expiration
Proof of vaccine series or completion and approval of client medical or religious exemption form (Preferred, not required)
Positive MMR titers ONLY from within the last 15 years
Completed within 1 year of start updated annually
Proof of full Hepatitis B series or at least two vaccines from the series completed or signed declination
Positive Varicella titers ONLY from within the last 15 years
Proof of vaccination within past 10 years, declinations not accepted
2-Step TB test with negative reads within the last 6 months

Additional Details
Primary Source Verification of professional license and / or certification, as required for the position ran within 30 days of start updated upon expiration. Attestations not allowed. Needs to be rerun every 2 years with RN license renewal & upon expiration. Temporary Licenses are NOT accepted – must have a valid permanent license.
