The Pharmacy Clinical Specialist - Pharmacogenomics Ambulatory coordinates and provides pharmaceutical care for patients within an assigned clinical area while participating in interdisciplinary rounds and collaborating closely with physicians, genetic counselors, and other healthcare providers to optimize medication therapy and improve patient outcomes. This role includes reviewing patient test results, making medication therapy recommendations, supporting pharmacogenomic-guided treatment decisions, participating in research initiatives, and precepting pharmacy students, residents, and trainees. In addition, the specialist contributes to strategic planning and innovative pharmacogenomic projects aimed at advancing personalized patient care and expanding the integration of pharmacogenomics data within the electronic medical record. The role also provides interdisciplinary education, including didactic teaching opportunities through the Cleveland Clinic Lerner College of Medicine and affiliated colleges of pharmacy.

 

A caregiver in this role works Monday-Friday, 8 am - 4:30 pm. No weekends or holidays.

 

A caregiver who excels in this role will: 

  • Perform pharmacotherapeutic monitoring and provision of pharmaceutical care for patients in the area of service responsibility in a routine, continuous manner, taking into account the age and clinical condition of each patient. 

  • Provide general and patient specific drug information to patients, physicians, nurses and other healthcare professionals. 

  • Interact routinely with physicians, nurses and other healthcare professionals to provide optimal pharmaceutical care to patients. 

  • Support clinical pharmacist and technician educational development by participating in departmental staff development programs and provide leadership in the education of staff involved in providing pharmaceutical care to patients. 

  • Support clinical pharmacists in providing pharmaceutical care to patients by direct interaction and formal educational programs. 

  • Support and participate in pharmaceutical related research in area of specialty. 

  • Maintain expertise in areas of practice through continual learning, journal review and other educational activities. 

  • Educate clinical pharmacists, technicians, pharmacy students, post-graduate trainees, allied health professionals, nurses and physicians in pharmacotherapy within the specialty area. 

  • Provide involvement in strategic planning and innovative pharmacogenomics projects to further patient care.

  • Engage in expansion of use of pharmacogenomics data to guide patient care in our electronic medical record.

  • Manage our ambulatory PGx clinic including pre-test counseling and results discussion with patient and their caregivers with opportunities to work under a collaborative practice agreement.

  • Conduct and analyze outcomes-based PGx research and collaborate on implementation of science projects.

  • Educate other healthcare professionals and precept pharmacy students and residents.

  • Provide interdisciplinary education which includes didactic opportunities at Cleveland Clinic Lerner College of Medicine and affiliated colleges of pharmacy

 

Minimum qualifications for the ideal future caregiver include: 

  • PharmD or Bachelors of Science degree in Pharmacy from an accredited college of pharmacy and completion of a post-graduate residency or fellowship training 

  • Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current State within 90 days of hire 

  • Completion of a post-graduate year 2 residency (PGY2) or fellowship required OR completion of a post-graduate year 1 residency (PGY-1) plus minimum of 2 years of experience as a clinical specialist or equivalent role, which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area OR minimum of 4 years of experience as a clinical specialist or equivalent role which includes providing pharmaceutical care and performing pharmacotherapeutic monitoring including 50% or more of time spent in the specialty area 

  • For individuals required to administer immunizations, current Basic Life Support (BLS) certification through American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire 

  • For individuals required to respond to adult or pediatric cardiopulmonary arrest events, current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certification through American Heart Association (AHA), or certification obtained within one (1) year of hire

Physical Requirements:

  • Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time. 
  • Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
  • Position often includes reaching, stretching, stooping, climbing steps and sitting.
  • Must have normal or corrected vision.
  • Must have ability to clearly communicate verbally by phone or in person

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required

Pay Range

Minimum Annual Salary: $101,900.00

Maximum Annual Salary: $155,390.00

The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.). The pay range does not include the value of Cleveland Clinic’s benefits package (e.g., healthcare, dental and vision benefits, retirement savings account contributions, etc.).

Salary : $101,900 - $155,390
