Clearwater Assisted Living, LLC is a rapidly growing assisted living organization with core competencies in assisted and transitional care throughout Wisconsin. We serve adults with disabilities in the greater Eau Claire community. Our primary staff are Direct Support Professionals who assist our clients in-home offering assistive services. The positions we offer range from Direct Support Professionals to Program Managers.
General Role Description:
We are currently looking for a Part Time Direct Support Professional to provide personal care assistance for our clients approx. 20 hours per week. The shifts are 5pm - 9pm Monday through Friday. The ideal candidate should be independent, empathetic and thorough on the job with a strong commitment to helping others, and should have Hoyer transfer experience.
Responsibilities:
- Client appointments, client cancellations or client updates that should be noted or addressed
- Consistently meet the expectations of completing client logs daily and reporting accurate mileage logs
- Follow the Individual Care Plan (ICP) for each client
- Uphold a high standard of care by showing up to work each day on-time with a positive attitude and integrity to work with your clients in a diligent manner
- Follow our scope of services and provide assistance as needed for encouraging and reminding clients to complete activities of daily living
Skills:
- Ability to think ahead and be proactive - problem solve as client challenges arise
- Work independently with a population of individuals with physical and developmental disabilities
- Ability to follow detailed training instructions on personal cares
