Habilitation Aide – Fairfax ICF
Clearbrook has three Intermediate Care Facilities (aka ICFs), each housing multiple clients. Goals are developed for each client based on their needs. Programming is assigned, usually pertaining to areas such as self-care, academics, community living & integration, as well as various workshop skills. Each facility has a nurse on staff at least 20 hours per week to coordinate any medical needs required by the clients.

Location: Rolling Meadows, IL
Hours: Part-Time
Schedule: Monday/Tuesday: 6:00am-10:00am & Sunday 9:00am-9:00pm (20 hrs/wk)
Pay: $21.35/hour Paid Training!
Med Passer Differential: $1/hr during medication pass periods
Weekend Pay: $3/hr differential
Responsibilities:
- Responsible for the direct provision of active treatment to the clients including training and assistance in all areas addressed in the individual program plans of the clients to whom she/he is assigned.
- Teach and provide the support necessary for the individual’s use of community resources.
- Assist clients with daily living activities including (but not limited to) personal hygiene, feeding & housekeeping.
- Assist clients in all areas of social and recreational program activities for the day.
Requirements:
- High school diploma or equivalent
- Valid IL driver’s license required
- Must be at least 21 years of age
*previous experience with behavior disorders and autism related issues preferred.
