Clean Recovery Centers is expanding and we are looking for an experienced addiction/mental health Case Manager to start as soon as possible.
A bachelors or master's degree is preferred. 2 to 3 years of addiction/mental health case management experience is preferred. Case Management experience is required.
The ideal candidate will have a strong work ethic and a passion for helping those with addiction/mental health challenges.

The Case Manager is responsible for performing client care activities as directed by the Clinical Director. The
Case Manager is responsible for managing an assigned caseload of clients having substance use disorders, for
assessing client needs, and for coordinating basic needs services for clients from admission through discharge.
The primary focus is job seeking assistance and housing assistance for clients. The Case Manager collaborates
with the Treatment Team in the formulation of the Discharge Plan, and assists in coordinating discharge
planning.
