Job Summary
We are seeking a compassionate and dynamic part-time Licensed Social Worker to join our adult day services dedicated team. The Social Worker plays a vital role in supporting family caregivers and adult day care members by addressing psycho-social, emotional, and economic needs. This position ensures holistic care through assessments, resource coordination, emotional support, education, and advocacy, fostering a positive and supportive environment for both caregivers and members. This is an exciting opportunity to make a meaningful impact in the lives of older adults living in the community and their family caregivers. The position offers a flexible schedule with no weekends, holidays or on-call.
Key Responsibilities
- Conduct comprehensive assessments of family caregivers to identify psycho-social and economic needs.
- Develop individualized care plans in collaboration with caregivers.
- Identify and connect families with appropriate community resources.
- Make referrals for services such as counseling, financial assistance, and respite care.
- Teach caregivers the importance of self-care, respite, and healthy coping strategies.
- Provide routine check-ins, one-on-one support, and monitor for signs of burnout.
- Assist families with completing Advanced Directives and guardianship applications.
- Support Medicaid applications and other government forms.
- Plan and lead caregiver support groups and retreats to foster community and resilience.
- Guide families through transitions when members move to long-term care facilities.
- Interact daily with members to identify needs, stress triggers, and behavioral changes.
- Offer active listening and emotional support to members.
Qualifications
- Education: Bachelor’s or Master’s degree in Social Work (BSW/MSW) from an accredited program.
- Licensure: State licensure or eligibility required.
- Experience:
- Minimum of 2 years of experience in social work, preferably in adult day care, geriatric care, or community health settings.
- Skills:
- Strong interpersonal and communication skills.
- Knowledge of community resources, Medicaid processes, and caregiver support strategies.
- Ability to facilitate groups and provide crisis intervention.
