We are seeking an Advanced Practice Provider (Nurse Practitioner or Physician

This is not a traditional primary care role. It is an opportunity to practice at the
intersection of medicine, prevention, and performance.
Role Overview
As an Advanced Practice Provider at Clay, you will serve as a core clinical leader within a multidisciplinary care team. You will build longitudinal relationships with patients, guide them through their health journey, and deliver high-touch, data-informed care focused on measurable outcomes.
You will work alongside physicians, health coaches, and Clay Guides to create a
coordinated and elevated patient experience.
Key Responsibilities
Clinical Care & Patient Experience
- Deliver comprehensive, patient-centered care across acute, chronic, and preventive needs
- Lead longitudinal care relationships with a defined patient panel
- Conduct in-depth assessments, including lifestyle, metabolic, and longevity-focused evaluations
- Develop and execute personalized care plans aligned to patient goals
Proactive & Preventive Health
- Focus on early detection, risk reduction, and health optimization
- Interpret and apply advanced lab testing and diagnostics
- Guide patients on nutrition, movement, recovery, and behavioral health strategies
- Incorporate evidence-based longevity and functional medicine principles where appropriate
Care Coordination & Team Integration
- Collaborate closely with physicians, health coaches, and Clay Guides
- Ensure seamless coordination across referrals, diagnostics, and follow-up care
- Participate in care planning and case reviews
Technology & Data-Driven Care
- Utilize EHR and digital health tools to track patient progress
- Monitor key health metrics and adjust care plans proactively
- Contribute to building scalable, tech-enabled care pathways
Patient Engagement & Education
- Build strong, trust-based relationships with patients
- Educate patients on proactive health strategies and available services
- Support adherence and long-term behavior change
Requirements
- Active Nevada license as an Advanced Practice Provider (Nurse Practitioner – APRN or Physician Assistant – PA-C)
- Minimum 3 years of clinical experience (primary care, urgent care, or related setting preferred)
- Experience managing a broad range of patient conditions across age groups
- Comfort using EHR systems and digital health tools
- Availability: In-person at our Las Vegas clinic, Monday – Friday, 8:30 AM to 5:00 PM PST
