What are the responsibilities and job description for the Registered Dietitian position at Claxton Dietetic Solutions?
Claxton Dietetic Solutions, LLC is seeking a motivated Registered Dietitian to join our team supporting residents in skilled nursing and rehabilitation communities throughout Northeast Ohio. This flexible part-time position offers approximately 24 hours per week, with an additional quarterly assisted living visit (approximately 4 hours per quarter).
This position is ideal for an RD who enjoys working independently while collaborating with an interdisciplinary care team to provide high-quality nutrition care and support positive resident outcomes.
Key Responsibilities
- Complete comprehensive nutrition assessments and documentation
- Develop and update individualized nutrition care plans
- Complete MDS documentation and nutrition-related care planning requirements
- Monitor resident nutritional status, weight trends, and clinical outcomes
- Participate in interdisciplinary meetings and nutrition risk reviews
- Conduct meal observations and dining rounds
- Perform food service and sanitation reviews as needed
- Provide nutrition education and support to residents, families, and staff
Qualifications
- Registered Dietitian credential through the Commission on Dietetic Registration (CDR)
- Current state licensure/certification, as required
- Strong assessment, documentation, and communication skills
- Ability to effectively manage an independent schedule
- Experience in long-term care, rehabilitation, or geriatric nutrition preferred, but not required
