- Variety in your day with a meaningful voice in client care
- Benefits, including health, dental, and vision insurance; 401k
What you'll do:
- Complete initial intakes/admissions to open new cases
- Complete periodic nursing assessments in clients' homes
- Develop and update individualized plans of care
- Make supervisory visits and provide clinical guidance to caregivers
- Monitor changes in clients' needs and coordinate with families, the client's providers, caregivers, and the office team
- Keep clinical documentation accurate and current
What we're looking for:
- Your current New York State RN license in good standing
- Strong clinical judgement, communication, and organizational skills
- A warm, practical approach to working with older adults and their families
- Comfort working both independently in the field and collaboratively with a team
- Strong commitment to follow up and timely communication
- A valid driver's license and reliable transportation
- Home care, case management, and/or community nursing experience preferred
This is a full time field and office position, generally 8:30 am to 4:30 pm Monday through Friday.
