Summary
This position is responsible for the provision of individual and family therapy sessions for consumers assigned to each caseload. This position also ensures completion of required clinical documentation related to individual caseloads in a timely manner.
Essential Duties And Responsibilities
- Demonstrates proficiency with assessment skills by completing psychosocial assessments at in-take and in on-going intervals as outlined by program requirements; completing standardized assessments and/or outcome based measures.
- Developments individualized treatment plans by adhering to applicable county, state and federal standard regulations when developing individualized treatment plans for consumers
- Provision of therapy to consumers in individually recommended setting by providing therapy in recommended setting by adhering to recommendations outlined in individual consumer treatment plans
- Provision of treatment of the whole person by providing service in collaboration with all team members involved with the consumer, and with the understanding that treatment of the whole person is crucial
- Evaluates individualized consumer-drive treatment plan reviews by completing on-going evaluation of efficacy of treatment evaluations, via development of treatment plan reviews
- Develops transition and discharge planning by evaluating recovery and progress through treatment goals, symptomology
- Completes all required clinical documentation by completing progress notes per service, including necessary signatures, discharge summaries, incident reports, treatment reviews via electronic health record system
- Responds to emergency/crisis situations with team members and consumers by committing to ensuring the safety of the consumer, and the proper utilization of de-escalation skills, when needed
- Maintains assigned, planned schedule by following program guidelines to ensure regular service provision and maintenance of caseload, per agreement with program supervisor
- Manages and supports coordination of care through routine conversations and referrals when needed (i.e. natural supports identified by the consumer, outside referrals, PCPs, school districts, etc.)
- Maintains a commitment to professional development by participating in on-going supervision, training, and department and team meetings, as outlined by policies and procedures
- Adheres to professional and ethical interaction policies and procedures by interacting professionally, ethically, and respectfully with consumers, other team members – including family members, and co-workers, at all times; following policy and procedure and maintaining appropriate professional boundaries
- Supports on-going and future business needs by completing other duties, as assigned, to effectively support business priorities and processes

- Home, school, community setting or outpatient clinic – as outlined in individual consumer treatment plan
- Up to 100% of travel between appointment sites or other locations – as outlined in program requirements
- Master’s degree from an accredited college or university in Psychology, Social Work, Counseling, or similar clinical discipline.
- Must have a verified clinical practicum
- Or licensed (LPC, LCSW, LMFT) in the state of Pennsylvania
- At least 1 year of experience with either a CASSP service system, or employed by a licensed mental health services agency, or subcontracted agency, or other mental health clinicians with documents training in the field of behavioral modification techniques
