Foster Care Coordinator

Summary

The Care Coordinator, under the supervision of a Program Director, Assistant Program Director, or Lead Therapist, provides Camelot-approved therapeutic interventions, case coordination and/or resource linkage to clients and other involved parties (i.e. family, DCS, Court personnel, non-relative supports, etc.) to effect identified needed changes within the individual or family.

Essential Duties And Responsibilities Include

• assessing clients on intake as needed; completing clinical documentation within established time frames
• attending weekly Treatment Team meetings and making presentations on each assigned client;
• working with the team to create, personalize, and update all Treatment Plans and other necessary documentation, and assuring that plans are consistent with Camelot policy, state licensing regulations, and COA accreditation requirements;
• serving as a liaison with other professionals and agencies involved in the client’s care;
• assuring integrity and excellence by completing peer audits as assigned; rectifying all chart audit deficits within the allotted
• time frame; completing all on-call duties;
• contributing to the Performance Improvement Cycle, including the collection and use of data to continuously improve client and program services;
• and supporting fellow team members to ensure the best outcomes for our clients and families.
  
  

Program-Specific Responsibilities

Therapeutic Foster Care

In addition to the general responsibilities listed above, the Therapeutic Foster Care Care Coordinator is also responsible for the following services: providing supervised visits and client-centered and strength-based individual, group, and family therapeutic interventions as outlined in the Treatment Plan; providing ongoing support and assistance to foster families; fostering sustainability by implementing discharge planning within the established time frames; participating in all necessary appointments for clients and families (school, DCS, court, medical, psychiatric, etc.); involving families in the treatment process to encourage reunification when possible; linking the client and family with specific services and resources necessary for success after discharge; monitoring medication management no less than bi-monthly while the child is in the foster home; and other duties as assigned, including any appropriate foster parent recruitment activities.

Prevention

In addition to the general responsibilities listed above, the Prevention Care Coordinator is also responsible for the following services: providing client-centered and strength-based preventative treatment and interventions to help clients avoid a higher level of care including state custody, juvenile detention, Care Coordinator I and/or hospitalization; establishing rapport with family members to effect identified changes necessary for sustained success; developing relationships within the community for both client and family resources, as well as referral resources; marketing to new referral sources and seeking out new opportunities for prevention services in the community; communicating with referral sources regarding client progress while maintaining confidentiality; being available to families in crisis; and other duties as assigned.

Qualifications

• At least 1 years of relevant experience in the Mental Health Field. A Bachelor’s degree in a Human Service discipline from an accredited four-years college or university is required for this position.
• experience working with children/ adolescents in a therapeutic treatment environment; good organizational skills
• ability to communicate effectively and work flexible hours consistent with program needs
• a licensed driver with automobile insurance in good standing and access to reliable transportation at all times
• sensitive to other cultures and socioeconomic levels
• is CPR and First Aid certified, or be willing to obtain certification before working with clients.
