TITLE: Trained Medication Aide (TMA)
REPORTS TO: Director of Nursing and Licensed Charge Nurse
Job Summary:
Function as team leader in assuring that all cares are completed in such a way as to provide quality care. Administer medications according to Clarkfield Care Center policy for TMA.
Qualifications: NA/R qualifications. TMA course completion. TMA skills passed. Must know, understand and use Standard Precautions. The TMA is also required to have an ability to read, write and speak English, and complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.
General Responsibilities:
- Receive report from nurse going off duty, getting full information on resident’s condition and new orders.
- Administers, prepares and charts medications according to established policies and procedures: oral, rectal, topical, eye drops and eardrops.
- Administers PRN medications after consultation with a nurse.
- Performs resident treatments as has been trained.
- Makes rounds periodically on each resident on her team.
- Refers residents needing assessment or special monitoring to a nurse who assumes that responsibility. Example: Physician’s Plan of Care
- Informs ordering person of needed nursing supplies.
- Delivers resident care as needed.
- Maintain good housekeeping in med room and cupboards.
o Clean for treatment.
o Wiping medication bottles after use.
o Watching for smeared or illegible labels and having them relabeled by pharmacist.
- Assists in dining room at mealtime.
- Maintains all required documentation in an accurate and timely manner per policies and procedures including medications, treatments, and weekly charting.
- Observes, records and reports to a nurse regarding residents conditions, reaction to drugs and treatments, and significant events.
- Assist with admission, transfer and discharge responsibilities not related to assessment.
- Any other duties requested by nurse in charge within scope of TMA.
- Attends required in-services and staff meetings.
- May do routine blood sugar checking. The LPN will check their own blood sugars if they need to follow the check with insulin administration.
- Discussion with family of resident’s physical condition, medications or behavior should be referred to the nurse in charge, conveying the philosophy of the facility to family and friends of the residents.
- Disciplinary action of team members should involve the nurse in charge.
- Maintains current CPR certification.
- Apply ace/elastic bandages.
- May give:
o Oral meds including sublingual and routine controlled drugs.
o Pain and behavior prn meds only after conferring with nurse. Chart “per charge nurse” on EMR.
o PRN Narcotics only after conferring with a nurse. Chart “per charge nurse” on EMR Record. Additionally, chart administration of “scheduled II narcotics” in the narcotic logbook.
o Lanoxin with pulses.
o Eye and ear gtts. Eye ointments.
o Topical cream and ointments.
o Patches. If “scheduled II narcotic” patch, such as Duragesic (fentanyl) – chart administration in narcotic logbook.
o Rectal suppositories.
o Inhalation meds.
o May crush medication, charting amount of food, given if drug was added to food.
o Anticoagulants.
- May not give or do:
o Injections, meds via n.g. or g. tube, vaginal/urethral routes.
o PRN O2 due to necessary data gathering prior to administration.
o May not do TXs on stage III or IV decubitus ulcers or fresh surgical wounds. No sterile TXs. No sterile catheter procedures.
o May not score tablets.
o May not take verbal orders from doctor or transcribe orders.
o Neuro checks.
o Medicare charting.
o May not do blood sugar checking for insulin dosing.
