TITLE: Home Care LPN/RN

REPORTS TO: Director of Nursing

 $25.00-$41.00 wage range

Job Summary: The Home Care Nurse will provide skilled nursing services and care coordination for home care clients to ensure safe, effective, and compliant care. Responsible for coordinating client admissions and discharges, completing OASIS assessments and updating plans of care (RN only). LPN would be assisting by collecting and reporting assessment data.

Qualifications: Two years of previous experience with nursing scheduling and health information preferred. Must have a current/active LPN or RN license with the State of Minnesota. Must keep up with CEU’s for license renewal. The Home Care Nurse is also required to have an ability to read, write and speak English, and complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.

General Responsibilities:

• Ensuring authorizations are current.
• Conduct or assist with Oasis assessments and update client Plan of Care.
• Assist with new hire orientation.
• Staff training and education as required annually.
• Ensure nursing staff complete chart reviews.
• Maintain and coordinate Home Care and Assisted Living schedule to ensure adequate/appropriate staffing.
• Contacting Primary Care Provider's & Pharmacy as needed.
• Schedule client medical appointments.
• Set up client medication and administer medication, including but not limited to monthly injections, managing client glucose meters, etc.
• Unpack supplies, stock storage area, maintain storage order, and label storage areas.
• Put information into charts after admission and file discharged charts.
• Assist with emergency transfer/ discharge papers/ copying and faxing as needed.
• Responsible for the overall service and quality of care provided.
• Coordinate survey initiated by the Department of Health
• Assist in investigations regarding Vulnerable Adult issues and appropriate reporting if needed.
• Manage the medical treatment and related services executed by nursing personnel.
• Coordinate and establish and/or delegates policy development for client care plans, standards of practice, and agency policies and procedures.
• Responsible for learning, understanding, and implementing legal and regulatory requirements as it affects clients, their care and staff.
• Foresee, establish, and/or communicates changes to policies and procedures to meet internal or external demands.
• Seek, recommend, and/or implement new methods and means of providing quality service and continuous improvement.
• Follow and abide by the HIPAA (Health Insurance Portability and Accountability Act) privacy policy as administered by Clarkfield Care Center and Valhalla.
• Notify Director of Human Resources to ensure recruitment advertising gets completed.
• Other duties as directed.

Salary : $25 - $41
