Job Purpose
The Outreach Supervisor will have supervisory responsibilities for the staff serving in outreach positions.
Key Responsibilities
· Provide direct supervision and support to designated staff as assigned by the Director of Client Access, including regular individual supervision meetings, coaching, and performance evaluations.
· Ensure outreach services are delivered in alignment with trauma-informed care principles and organizational values.
· Monitor documentation for accuracy, timeliness, and compliance with agency and regulatory standards.
· Assist staff in problem-solving barriers to care, including coordination with internal and external providers, systems, and natural supports.
· Facilitate training and professional development opportunities for Outreach Workers to enhance skills in engagement, crisis response, and resource navigation.
· Review and analyze service data to identify trends, gaps, and opportunities for improvement in outreach efforts.
· Collaborate with leadership to develop and refine outreach protocols, workflows, and community engagement strategies.
· Serve as a liaison between the outreach team and other departments, ensuring effective communication and coordination of care.
· Support staff in managing high-risk situations, including crisis intervention and safety planning, and provide clinical consultation as needed.
· Participate in hiring, onboarding, and retention efforts for outreach staff.
· Maintain knowledge of community resources, programs, and behavioral health services to support staff and clients effectively.
· Embrace an understanding of the job duties and requirements for each outreach staff, including ERE, YBHL, CMHL, SCL, Peers and the management of the DM list.
· Run reports in the EHR and CareManager to ensure each employee is in compliance with employment expectations and provide additional support to employees when needed.
· Complete other duties as assigned by the Director Client Access or Chief Executive Officer.
Qualifications
Education: Minimum of a Master degree in Psychology, Social Work, or related health field.
Skills & Abilities: Possess strong leadership qualities. Effective oral and written communication. Aptitude to understand and flexibility to adapt to ever-changing regulations and requirements.
Working Conditions
General: Work is generally performed in office type settings where answering phone and using computers and other technology should be expected.
Special conditions -Meetings with others outside the office are common and may require ability to tolerate prolonged exposure to smoke and pets.
Physical Requirements
General: Job duties may require sitting or standing for long periods of time. Travel, with some trips requiring several hours in a vehicle, should be expected. Minor lifting and moving may be expected requiring ability to lift up to 20 pounds.
Special conditions -Meetings with others outside the office may require ability to enter and access homes/buildings of all configurations.
