Job Summary
Clark Community Mental Health Center is seeking a compassionate and dedicated Nurse Care Manager (NCM) to join our Population Health Team. This role is integral to our mission of providing integrated, whole-person care by bridging the gap between physical and behavioral health. The NCM collaborates with medical and behavioral health providers to support clients’ physical health, wellness, and recovery goals. Responsibilities include hospital follow-ups, health education, and data-informed care coordination to improve client outcomes.
Duties and Responsibilities
· Participate in treatment planning and support clients with self-management goals and wellness plans.
· Collaborate with Case Managers and healthcare providers to address health concerns and coordinate follow-up care.
· Monitor health reports for medication alerts, treatment changes, and hospital admissions/discharges.
· Track completion of required health screenings and assessments.
· Utilize online databases for client health tracking and data entry.
· Provide individual and group health education on chronic disease management, medications, and wellness.
· Educate staff and clients on medical issues and support wellness initiatives, including smoking cessation.
· Maintain accurate and timely documentation in Electronic Health Records (EHR).
· Collect and report data for quality improvement and performance measures.
· Advocate for client access to needed services and resources.
· Ensure compliance with HCH policies, Medicaid, and state/federal regulations.
· Serve as a medical resource within a multidisciplinary team and participate in team meetings and case reviews.
· Collaborate with behavioral health providers, external medical professionals, and managed care organizations to coordinate integrated care.
· Complete other duties as required by the position and/or as assigned by the immediate supervisor or the Chief Executive Officer.
Qualifications
Licensure: Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN), with an active license or license-eligible in the State of Missouri.
Experience: Prior experience in community mental health, primary care, or care coordination preferred.
Skills and Abilities:
· Strong interpersonal and communication skills.
· Knowledge of chronic disease management and medication monitoring.
· Proficiency in Electronic Health Records (EHR) or ability to quickly learn new computer systems.
· Comfortable using computers, with experience in Microsoft Office (Word, Outlook, Excel) preferred.
· Ability to work independently and as part of a collaborative care team.
Work Environment
· Primary setting: Office-based with standard office equipment (computer, phone, etc.).
· Travel expectations: Occasional local travel for client outreach, community events, state-wide NCM meetings, or coordination with healthcare providers.
· Home visits: Rare but may be necessary in special circumstances; candidates must be able to access homes of varying types.
· Physical requirements: Ability to sit or stand for extended periods; ability to lift up to 20 pounds; ability to tolerate exposure to pets and smoke when meeting clients in their homes.
