What are the responsibilities and job description for the Quality/Hedis Coordinator position at Clark Clinic?
Successful candidates will assist the clinic in leveraging patient outcome goals, following established standards and practices. Must have the ability to work in a fast-paced environment with excellent communication skills.
Work Schedule: Monday - Friday (8am-5pm)
Tasks:
- Scheduling members to be seen at least 4x per year All patients seen in-office scheduled for next visit
All canceled appointments and "no show" appointments are rescheduled
- Making Transition of Care phone calls Medical Record requests post-discharge (INP and ER)
Follow up in-office visit post-discharge (INP and ER)
- Scheduling new patients to be seen within 30 days of the effective date with PCP Prior records obtained
Use of new patient registration process
- HEDIS/Stars call outreach Part C Gap Report
Part D Gap Report
- Patient Referral coordination Concierge referral process to assist with the patient being scheduled with a specialist
Obtain records from specialist post-visit and load into the patient chart for PCP review (could reduce stat referrals bc of unobtained records)
- Telephone outreach to disenrolled members Assist with Medical Records transfers
