We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. Duties
- Conduct thorough assessments of residents’ clinical status, functional abilities, and care needs in accordance with federal and state regulations, including CMS guidelines.
- Complete and review MDS assessments accurately within designated timeframes to ensure compliance with all applicable standards.
- Collaborate with interdisciplinary teams—including nursing, therapy, social services, and medical staff—to develop individualized care plans based on assessment findings.
- Perform ongoing monitoring and updates of resident records to reflect changes in condition or care requirements.
- Facilitate accurate ICD-10 coding and CPT coding for billing purposes, ensuring alignment with clinical documentation and Medicare requirements.
- Support utilization review and management processes by analyzing patient data to optimize resource use and facilitate discharge planning.
- Maintain compliance with HIPAA regulations and uphold the confidentiality of all medical records and documentation.
- Must be able to be on call
Skills
- Extensive knowledge of MDS processes, including completion, review, and submission aligned with NCQA standards.
- Proficiency in Matrix (EMR system)
- Strong understanding of ICD-10 coding, CPT coding, ICD-9 coding, DRG assignment, and Medicare regulations related to skilled nursing facilities.
- Familiarity with hospital-based discharge planning, hospice care coordination, outpatient services, and home health environments.
