Overview
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. In this vital role, you will lead the comprehensive assessment and documentation of patient care needs within skilled nursing facilities, ensuring compliance with regulatory standards and optimizing patient outcomes. You will also be responsible for care plan coordination and medicare meetings. The small home like setting that we have allows for flexibility around your schedule. Duties

  • Conduct thorough assessments of residents’ clinical status, functional abilities, and care needs in accordance with federal and state regulations, including CMS guidelines.
  • Complete and review MDS assessments accurately within designated timeframes to ensure compliance with all applicable standards.
  • Collaborate with interdisciplinary teams—including nursing, therapy, social services, and medical staff—to develop individualized care plans based on assessment findings.
  • Perform ongoing monitoring and updates of resident records to reflect changes in condition or care requirements.
  • Facilitate accurate ICD-10 coding and CPT coding for billing purposes, ensuring alignment with clinical documentation and Medicare requirements.
  • Support utilization review and management processes by analyzing patient data to optimize resource use and facilitate discharge planning.
  • Maintain compliance with HIPAA regulations and uphold the confidentiality of all medical records and documentation.
  • Must be able to be on call

Skills

  • Extensive knowledge of MDS processes, including completion, review, and submission aligned with NCQA standards.
  • Proficiency in Matrix (EMR system)
  • Strong understanding of ICD-10 coding, CPT coding, ICD-9 coding, DRG assignment, and Medicare regulations related to skilled nursing facilities.
  • Familiarity with hospital-based discharge planning, hospice care coordination, outpatient services, and home health environments.

Pay: $25.00 - $40.00 per hour
