What are the responsibilities and job description for the Access Clinician position at Clara Martin Center?
Access Clinician: (Bradford, VT) We have an opening for a full time Access Clinician. This position works as a part of our centralized access team, interfacing with all agency programs to assist clients in making strong connections to treatment.
This position is based at our Bradford VT office, but some travel to other sites may be required. Essential functions of this position include the following:
- Conducts phone screening, risk assessment & therapeutic interventions by phone and in person to coordinate client entry to Agency programs.
- Gathers all required paperwork for fee setting, billing, insurance, prior authorization, including clinical / medical information from referral sources.
- Provide assessments, evaluations, case management and other brief treatment services to meet the demand on the services system.
- Intervenes in crisis situations to assess and stabilize client’s condition using available resources, providing safety and ensuring confidentiality.
- Performs care coordination and clinical documentation in an electronic medical record system.
A Masters degree is preferred but not required; candidates with a Bachelors degree in human services, psychology, social work or related field, and 2-4 years of experience will be considered. Clinical supervision towards licensure is available for Master Level staff.
