Responsibilities
- Assist patients with daily activities such as bathing, dressing, grooming, and mobility to promote comfort and independence.
- Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately in electronic health record (EHR) systems.
- Maintain patient confidentiality in accordance with HIPAA regulations while documenting medical records thoroughly and accurately.
- Use safe handling techniques and equipment such as Hoyer lifts to transfer patients safely, minimizing injury risk.
- Collaborate with nursing staff to observe changes in patient condition and communicate effectively about patient needs or concerns.
- Provide specialized care for individuals with conditions such as dementia, Alzheimer's disease, developmental disabilities, or other health challenges.
Skills
- Proven experience working with diverse patient populations including geriatrics, or individuals with disabilities, and compromised memories.
- Familiarity with EMR/EHR systems like Matrix Care for medical documentation-will train.
- Knowledge of medical terminology, anatomy, physiology, and aseptic techniques essential for safe patient care.
- Ability to perform vital signs measurement accurately .
- Strong caregiving communication skills to effectively support patients' emotional well-being and collaborate with multidisciplinary teams.
- Experience working in nursing homes setting helpful.
