We are currently seeking a Home and Community Based Services (HCBS) Coordinator to join our West Side team. This role is pivotal in planning, evaluating, and supervising the delivery of services approved through the Arizona Division of Developmental Disabilities (DDD).
Key Responsibilities
As an HCBS Coordinator, you will:
- Develop and implement individualized service plans in compliance with state and federal standards
- Build strong relationships with individuals, families, guardians, and agency personnel
- Conduct home/facility safety and compliance checks
- Respond effectively to emergency situations
- Evaluate care environments and ensure services meet DDD requirements
- Lead staff meetings and in-service trainings
- Participate in committees and attend relevant conferences/seminars
- Monitor licensure standards and ensure staff certifications are up to date
- Review incident reports and take corrective actions
- Maintain accurate documentation and prepare timely reports
- Perform other duties as assigned
Qualifications & Skills
Required:
- High school diploma or GED
- Minimum 3 years of experience working with individuals with intellectual and developmental disabilities
- Valid driver’s license, auto insurance, and Level One Fingerprint Clearance Card
- Ability to pass background check and pre-employment drug screening
- Physical ability to assist members and perform First Aid/CPR
- Ability to lift up to 50 lbs and sit/stand for extended periods
- Satisfactory driving record (MVR for current year and past 3 years)

- Human behavior and disability indicators
- Principles of developmental disability and mental health care
- Program development and implementation
- Licensing and safety standards in Arizona
- Community resources and social service agencies
- Report writing and behavioral treatment planning
- Team collaboration and multidisciplinary coordination
