Job Summary
We are seeking a dedicated and skilled Physical Therapist to join our team. The ideal candidate will be responsible for evaluating and treating patients with various physical conditions, developing individualized care plans, and ensuring optimal patient outcomes. This role requires a strong understanding of anatomy, physiology, and medical terminology, as well as experience in various settings such as home health care.
Responsibilities
- Assess patient conditions through comprehensive evaluations to determine appropriate treatment plans
- Develop and implement individualized care plans that address specific patient needs
- Provide therapeutic interventions including physiotherapy techniques, and orthopedic rehabilitation
- Collaborate with healthcare professionals to coordinate discharge planning and ensure continuity of care
- Educate patients and their families about treatment options, home exercises, and preventive measures
- Maintain accurate patient records using EMR systems to document progress and treatment outcomes
- Stay updated on best practices in physical therapy and participate in ongoing professional development
Qualifications
- Degree in Physical Therapy from an accredited institution
- Valid state license to practice as a Physical Therapist
- Strong knowledge of anatomy, physiology, medical terminology, and rehabilitation techniques
- Experience in acute care settings, geriatrics, outpatient clinics, or home health preferred
- Familiarity with discharge planning processes and care plan development
- Excellent communication skills for effective patient interaction and collaboration with healthcare teams
- Ability to work independently while also being a team player in a multidisciplinary environment
